Before completing this form, please ask your ASC administrator if your facility is a member of the Outpatient Ophthalmic Surgery Society (OOSS). If yes, you must register through the OOSS website, or ask your administrator to register you in order for you to receive the OOSS member discount (50% off the cost of each course).
